I acknowledge and understand that:

• I give permission for The Stage Door Performing Arts to collect, use and publish my image, name, or voice, or those of my child/ren (if under 18 years of age), for general publicity and public relations purposes, including photographs and videos used on websites, social media, marketing, promotional, or training materials.

• Photography and/or video may be taken during sessions for promotional and reporting purposes.

• Parents/carers are responsible for ensuring their child does not join in or participate during filming if they do not wish their child to be included.

• Classes are delivered in a public park environment at Bella Vista Pocket Park.

• A nominated First Aid Officer will be present at each session, and the dance instructor holds current First Aid and CPR certification.

• A basic first aid kit will be available at all sessions.

• Parents / carers are required to remain onsite for the duration of the class, and children are supervised at all times by both the parent / carer and the dance instructor.

• Classes operate in a clearly defined area of the park, and sessions may be paused or cancelled if weather, safety, or public conditions make it unsafe to continue.

• In the event of an emergency, emergency services may be contacted via 000 if required, and any incidents will be managed and documented as appropriate.

• I am responsible for informing The Stage Door Performing Arts of any relevant medical conditions relating to my child and for managing and providing any required medication during the session.

• As the legal parent or guardian, I release and hold harmless The Stage Door Performing Arts, its owners and operators, from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by the participant and/or myself while participating in classes, while on the premises, or while travelling to or from any premises under the control or supervision of The Stage Door Performing Arts.
